Care Cards

Which aspects of care are most important to you?

Care Cards support patients and their relatives to explore how the emotional needs and care preferences of patients can best be captured, monitored and addressed in real time as part of a quality-led care experience.

Care Cards outline eight aspects of care and can prompt discussion of preferences for care between staff and patients and their families and carers.

How the cards will be used

The cards outline eight different aspects of care:

  • Self confidence
  • Respect
  • Reassurance
  • Effectiveness
  • Safety
  • Comfort
  • Understanding
  • Honesty

Staff will explain the cards and ask you to put them in order of preference based on what is most important to you and how you want your care delivered

For example, if comfort is important to you, you choose ‘comfort’ as your priority card, and staff may then ask how you want your care to be delivered to ensure that this specific need is met.

Recording your preferences

If you are willing to take part in this project your responses, while confidential will be noted on a record sheet so that other staff involved with your care are able to read your views. Staff will also note any other preferences you talk about which may not be included on the eight care cards.

The cards will be used more than once during your care or stay in hospital and you may be asked if your preferences have changed and how your care can be improved to make sure that your needs are continuing to be met.
